Patient Care Coordinator
Key Responsibilities:
- Understanding the details of the special healthcare programs (Example: Diabetic program) to communicate with the patients effectively.
- Acting as a liaison between the patient and healthcare personnel by keeping the lines of communication flowing.
- Collecting all information about the treating patients, introducing the program to them, and registering those who are interested.
- Collecting the medical history, diagnosis, lab tests, and medications of all the patients registered in the said program.
- Scheduling patients for the concerned departments referred by the doctors.
- Educating patients about their healthcare options.
- Answering all patient calls, inquiries, in a compassionate and professional manner.
- Maintaining patient records up-to-date with strict confidentiality.
- Recommending doctors to the patients, by providing them with their educational background and expertise.
- Managing some financials and invoices.

Patient Relations Agent
Key Responsibilities:
- Serving as a focal point between the hospital departments and the clients.
- Maintaining a positive, empathetic, and professional attitude toward the clients at all times.
- Ensuring clients are properly greeted and welcomed at the reception desk.
- Providing accurate information as requested by clients at the desk or over the phone.
- Directing clients according to their respective queries/departments.
- Helping clients in rescheduling/modifications of appointments.
- Managing patient complaints in recording, and sending it to the concerned to resolve.
- Listen to customers’ concerns, issues and questions.
- Resolve customers’ concerns and answer customers’ questions to the best possible way.
- Following up with patients to ensure their inquiry or complaint has been satisfactorily resolved.
- Responsible for managing patient reward system.
- Responsible for maintaining a clean and safe work environment.
- Maintain a working knowledge of department policies and procedures.
- Secure everything at Information and Services desk at the end of each shift.
- Performing administrative and clerical duties, such as filing paperwork, when necessary.
- Others as assigned by the department manager.
