Position: Program Nurse
Position Purpose:
The Home Health Nurse provides comprehensive nursing care in a community setting to help patients achieve optimal health and quality of life while managing chronic diseases. The nurse delivers and manages effective nursing care to individuals and families through Patient Support Programs, adhering to Axios International policies and procedures and following local and international nursing standards and regulations.
Duties and Responsibilities:
Service Responsibilities:
The Home Health Nurse offers skilled nursing services based on physician orders and a care plan. Duties include:
- Telephonic follow-up consultations.
- Monitoring vital signs such as blood pressure, heart rate, temperature, and oximeter statistics during home care.
- Monitoring blood glucose levels and providing teaching and support for patients and families.
- Preparing medications as per physician orders.
- Administering medications, including injections, IVs, inhalation, oral, infusions, and rectal forms.
- Pain management and medication monitoring for patients with acute or chronic health conditions.
- Providing education and counseling tailored to the patient's treatment or care needs.
- Wound assessment and dressing changes.
- Delivering general nursing care.
- (Optional) Advanced Foot Care knowledge is a plus.
- Monitoring pain control and conducting cardiac assessments.
- Supervising and teaching health management to family members and caregivers regularly.
- Assessing mental health status.
- Completing Pharmacovigilance, Pharma, and Product Training, ensuring proper safety data collection and reporting.
- Reporting adverse events following local regulations and Axios SOPPs.
- Reporting concerns or changes in patient health to the program and/or treating physician.
- Familiarity with nursing equipment like ECG machines, spirometry, and monitors, along with the ability to provide patient assessments.
