Patient Care Coordinator

Requirements

• Full fluency in both English and Arabic (mandatory)

• University degree in a scientific or healthcare-related field

• 1–2 years of experience in the hospital or healthcare sector

Responsibilities

• Use Axios' Patient Management System (PMS) to oversee operations

• Coordinate with hospitals, pharmacies, and patients for smooth enrolment and treatment access

• Conduct field visits to doctors, pharmacies, and patients to ensure workflow continuity

• Update and maintain real-time patient records

• Prepare monthly patient data reports and quarterly quality assurance summaries

• Enhance operational efficiency through strong organizational skills

• Follow a patient-centric approach to drive positive outcomes

• Support administrative functions and undertake additional responsibilities as needed

Nurse Coordinator

Details

Requirements

• Bachelor’s degree in Nursing

• Minimum 2 years of healthcare experience

• Local Ministry of Health license or eligibility letter

• Excellent command of English

• Knowledge of advanced foot care is a plus, but not required

Responsibilities

• Deliver home health services in alignment with physician orders and patient care plans

• Conduct telephonic follow-ups and monitor vital signs (BP, pulse, temperature, oxygen levels)

• Track blood glucose levels and educate patients and caregivers

• Administer medications (injections, IV, oral, infusions, etc.)

• Provide pain management support for chronic or acute cases

• Perform wound care and dressing changes

• Evaluate mental and cardiac status as needed

• Participate in pharmacovigilance and training, and report adverse events as per SOPs

• Liaise with hospitals and stakeholders to implement support programs

• Conduct home visits and maintain real-time patient records

• Submit monthly monitoring reports and quarterly QA summaries

• Handle administrative tasks for the program and office

المهام

• التنسيق مع المستشفيات والصيدليات والمرضى لضمان سهولة التسجيل والحصول على العلاج

• تحديث ملفات المرضى بشكل فوري ودقيق

• إعداد تقارير شهرية حول بيانات المرضى وتقارير ربع سنوية لجودة الأداء

• خبرة لا تقل عن سنتين في بيئة صحية

• رخصة مزاولة المهنة أو خطاب أهلية من وزارة الصحة

• إجادة ممتازة للغة الإنجليزية

• معرفة بالعناية المتقدمة بالقدم ميزة إضافية وليست شرطاً

المهام

• تقديم خدمات تمريض منزلية حسب خطة العلاج وأوامر الطبيب

• المتابعة الهاتفية ومراقبة العلامات الحيوية (الضغط، النبض، الحرارة، الأوكسجين)

• مراقبة مستويات السكر في الدم وتقديم الدعم والتثقيف

• تجهيز وإعطاء الأدوية حسب تعليمات الطبيب (حقن، وريدي، فموي، إلخ)

• دعم إدارة الألم للحالات المزمنة أو الحادة

• تغيير الضمادات وتقديم العناية بالجروح

• تقييم الحالة العقلية والقلبية للمريض

• حضور تدريبات المنتجات والدواء والإبلاغ عن الأعراض الجانبية وفق الأنظمة

• تنفيذ زيارات منزلية وتحديث ملفات المرضى بشكل لحظي

• إعداد تقارير شهرية ومتابعة الجودة ربع سنوياً

• تنفيذ المهام الإدارية المرتبطة بالبرنامج والمكتب
