Home Health Nurse

Responsibilities

• Develop and Update Plan of Care
  Initiate, develop, and update the plan of care in accordance with the physician and other health professionals.

• Assess Patient's Physical and Emotional Status
  Assess patients’ physical and emotional status and safety factors. Update the patient observations chart during the home visit.

• Complete Evaluation Tasks
  Complete evaluation tasks, including reviewing medication and vital signs.

• Maintain Patient Documentation
  Maintain timely and effective documentation of patient medical records.

• Administer Medications and Tests
  Administer physician-prescribed medication, and conduct tests for insulin, blood pressure, glucose, urine, and stool.

• Wound Care
  Dress or redress wounds and assess the healing progress.

• Coordinate with Physicians
  Coordinate with physicians in the patient’s care plan as needed.

• Provide Palliative Care
  Keep the patient comfortable through palliative care.

• Recommend Tools or Devices
  Recommend tools or devices that may improve the quality of life for the patient.

• Address Family Concerns
  Listen to the concerns of family members and answer their questions.

• Educate Caregivers and Family
  Educate caregivers and family on the aftercare or ongoing care of the patient.

• Provide Suggestions for Improved Healthcare
  Provide suggestions for improved healthcare to physicians and family members of the patient.

• Implement Safe, Competent Care
  Implement safe, competent care with home care patients and families within the homecare program policies, procedures, and standards of care.

• Follow Infection Control and Safety Policies
  Follow infection control and safety policies and procedures.

• Display Professionalism
  Display a professional attitude and behaviors of trust, respect, honesty, and caring.

  تنفيذ رعاية آمنة وكفؤة مع المرضى وأسرهم ضمن سياسات وإجراءات ومعايير رعاية البرنامج المنزلي.

• اتباع سياسات التحكم في العدوى والسلامة
